Use this form to report to the DHC National Office, information about the project completed during the 3-month period of study and reflection. (Remember the project for membership should be very brief. See Chapter IX of the Candidates Study Manual for details.)
To complete the Project Report form click:

This form is to be used by the Mentor/Convener/Teacher (or designee) to keep track of attendance of each person in the training using the Candidates Study Manual. The Manual is to be completed during the 3-month period of study and reflection. The form is for local use only.
To complete the Training Session Attendance form click:

This form is to be filled out by each member. The member is to indicate on the form what they want done with their cross or crosses should they for whatever reason no longer be a member of the order. This form should be kept with local documents.
